Provide one-on-one community-based support to individuals with developmental disabilities to increase their independence and inclusion. Responsibilities include implementing Staff Action Plans, documenting services in electronic health records, and ensuring the health and safety of clients.
Candidates must have a high school diploma or GED and a valid driver's license with a clean driving record. Possession of a personal smartphone for documentation purposes is strongly preferred.
Responsible for the health and safety of the individuals supported Provide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community Participate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan Follow objectives as described in the person’s Staff Action Plan and as discussed with supervisor Attend training to improve services provided Complete job requirements (training, PPD, etc) within appropriate time frames Report any potential incident to the designated Family Support Coordinator/Supervisor immediately, or the day that it occurred. Accurately enter time and services into Racker’s electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision) Use of smart-phone for documentation purposes required. Ownership/usage of personal smart-phone is strongly preferred Communicate regularly with supervisor regarding service provision Attend scheduled supervisory meetings regularly Uphold standards of professionalism with regard to behavior and dress Assume responsibility for related duties as required or assigned
