Nurse Practitioner, Cardiology

• Opportunity Information Sutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner to join our Cardiology team at 1580 Valencia Street in San Francisco, California.
• This role supports two cardiologists in a growing outpatient practice and is essential to improving patient access and care continuity. --- About the Opportunity The NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases.
• The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure.
• A key part of this role includes educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications to support long-term health.

Qualifications

Education: · Graduate of an accredited Nurse Practitioner (NP) program Licensure & Certifications: · Current California NP license · DEA Controlled Substance Registration Certificate (CSRC) · Current BLS and ACLS certification Experience: · Experience in cardiology or internal medicine preferred · New graduates with a strong interest in cardiovascular care are welcome to apply Organization Details Sutter West Bay Medical Group (SWBMG) is a premier multi-specialty medical group made up of over 400 physicians practicing in the greater San Francisco service area.
