The Nurse Practitioner will manage follow-up visits for established cardiology patients and support transitional care efforts through timely post-discharge visits. A key part of this role includes educating patients on cardiovascular conditions and treatment plans.
Candidates must be graduates of an accredited Nurse Practitioner program and hold a current California NP license. Experience in cardiology or internal medicine is preferred, but new graduates with a strong interest in cardiovascular care are welcome to apply.
