The physician will focus on optimizing patients’ overall health by assessing health risks and providing disease prevention, education, and detection. Duties include medical care with disease management and working with a team to deliver high-quality comprehensive care.
Candidates must hold an MD or DO degree and be credentialed by the Indiana Medical Licensing Board prior to hire. Advanced Life Support certification is preferred.

Physician MD/DO credentialed from the Indiana Medical Licensing Board obtained prior to hire date or job transfer date required. Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
