The Community Health Coordinator supports outreach, health education, care coordination, and administrative functions to enhance patient outcomes, primarily by engaging Enhanced Care Management (ECM) Members and community patients. This role involves conducting outreach, delivering culturally appropriate health education, coordinating necessary services like appointments and transportation, and managing administrative tasks such as referral assignment and reporting assistance.
Candidates must possess a High School Diploma or AA degree, with prior experience working with at-risk families in health care or human services preferred, alongside strong knowledge of community resources. Essential requirements include being bilingual in English/Spanish, proficiency in computer skills including intermediate Excel, and the ability to maintain professionalism, confidentiality, and work both independently and collaboratively.

The Community Health Coordinator will report directly to the Enabling Services Manager and will be responsible for supporting outreach, health education, care coordination, and administrative functions to improve patient outcomes. The Community Health Coordinator will provide engagement with Enhanced Care Management (ECM) Members and community patients, deliver health education, coordinate care services, and manage administrative tasks such as assigning referrals and assisting with reporting. The Community Health Coordinator would need to perform all duties with independence, professionalism, precision, and confidentiality.
This position is funded through the CITED grant and is currently approved through December 2026. Continued employment beyond the current grant period is contingent upon Harbor Community Health Centers securing continued funding.
