The Physical Therapist will provide individualized skilled care to patients in their homes, which includes assessment, planning, implementation, and evaluation of treatment programs based on patient diagnosis and age. Responsibilities also involve collaborating with the healthcare team, educating patients and families, and participating in agency program development and performance improvement initiatives.
Candidates must be a graduate of an accredited Physical Therapy program and hold licensure in the state of practice (NC/SC) along with CPR certification, requiring a minimum of one year of experience. Essential requirements include physical capabilities for lifting and transferring patients, knowledge of home health procedures, and the ability to apply clinical principles effectively.

Provides individualized patient care for patients in all developmental stages throughout the life span including: Neonates - 0-28 days, Infant - 1-12 months, Child - 1-12 years, Adolescent - 13-17 years, Adult - 18-72 years, Geriatric - 72 + years, according to established policies procedures, and guidelines. Participates in professional responsibilities such as program development, student programs, and performance improvement programs. Provides patient consultation and assumes communicative role with other professionals. Refers patients to other professionals as appropriate and ordered. Performs other duties as assigned. PRIMARY JOB DUTIES 1. Assesses, interprets, plans, implements, and evaluates patients with appropriate level of skill given the patient’s age and diagnosis. Admits patients to the agency when P.T. is a primary skilled need. 2. Assists with program development and discipline specific operations efficiently and productively. 3. Appropriately communicates and documents information in accordance to agency policies and procedures and discipline-specific guidelines. 4. Contributes to program effectiveness. 5. Organizes and performs work effectively and efficiently. 6. Maintains and adjusts schedule to enhance agency performance. 7. Demonstrates a daily commitment to the values of the agency. 8. Demonstrates positive interpersonal relations in dealing with all members of the agency. 9. Maintains and promotes customer satisfaction. 10.
