The Physical Therapist will connect with patients in their homes to gain insight into their living situations and assist them in achieving independence within their own environment. This role is part of a patient-centered approach ensuring a meaningful difference is made regardless of location.
Candidates must possess a current Physical Therapist license or the ability to obtain one in the state of practice, along with a minimum of one year of experience in a healthcare setting, with previous home health experience being preferred. Applicants must also have a valid driver's license, auto insurance, and current CPR certification.
