The role involves providing psychosocial support to individuals, families, and groups coping with illnesses by conducting social work assessments, coordinating care, and making necessary referrals. Responsibilities also include educating patients and families on community resources and reinforcing the established plan of care through counseling and conferences.
Candidates must be licensed by the Wyoming Mental Health Professions Board and complete the Cheyenne Regional AHA RQI within 14 days of starting. Desired qualifications include two years of social worker experience and background in grief and bereavement.

Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
