Position Overview The in-home Physical Therapist will evaluate, develop, and implement personalized treatment plans for clients, focusing on improving mobility, reducing pain, and enhancing quality of life. This role requires excellent interpersonal skills, clinical expertise, and the ability to work with diverse patient populations, including high-profile clients. The ideal candidate is compassionate, detail-oriented, and committed to achieving positive outcomes for clients while adhering to the highest standards of patient care and confidentiality.

Key Responsibilities Patient Care

• Conduct comprehensive physical therapy evaluations to assess patient conditions, needs, and goals.
• Develop and implement individualized treatment plans, incorporating therapeutic exercises, manual therapy, and modalities as appropriate.
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
• Educate patients and their families on exercises, home programs, and injury prevention strategies.

Collaboration

• Work closely with physicians, nurses, and other healthcare providers to ensure coordinated and comprehensive care.
• Provide progress reports and communicate treatment adjustments to the care team as necessary.
