Assist with activities of daily living, household cleaning, meal preparation, shopping, budgeting, personal goals, appointments, community activities, and engagement.
Maintain a clean, safe, and supportive living environment.
Promote independence, personal choice, and life skills development.
Support individuals in achieving goals and objectives outlined in their Individual Service Plans (ISPs) and document progress accordingly.
Observe and communicate changes, concerns, and progress to supervisors and team members.
Transport individuals to appointments, activities, and community outings as needed.
Complete accurate and timely electronic documentation in accordance with program and regulatory requirements.
