The Critical Time Intervention (CTI) Case Manager stabilizes housing retention for recipients of Intensive Case Management Services (ICMS) in project-based developments. Responsibilities include regular tenant meetings for strengths-based case management, service coordination, and collaborating on person-centered plans to achieve short and long-term goals.
Candidates must be willing to provide community and in-home based services, possess high tolerance for individuals with complex needs, and exercise mature judgment while working independently and collaboratively. Preferred qualifications include two years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
The following education and professional experience is preferred: Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.
