The Community Health Worker is tasked with identifying high-risk prenatal and postpartum women and assisting them in accessing essential primary and preventive health services. This assistance includes connecting individuals with services such as prenatal care, PCAP, WIC, and other necessary supportive resources.
Candidates must possess a High School Diploma or equivalent and must have a valid New York State Driver's license, a personal car for work duties, and active automobile insurance. Basic computer skills along with good writing and verbal communication abilities are also required.

High School Diploma or equivalent. Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy. Basic computer skills. Good writing and verbal skills. Spanish or Creole speaking a plus.
Usd $22.05 - Usd $22.05 /Hr.
