The Licensed Nursing Assistant performs delegated nursing and homemaking duties for patients in their homes. They report patient needs and changes in condition to the appropriate case manager and adhere to care plans.
Candidates should have a high school diploma or equivalent and a current Nursing Assistant License in New Hampshire. Six months to one year of experience in a related field is desired.

About the Job: Performs delegated nursing, homemaking, and other delegated duties for patient/clients of all ages and families in their home or other designated facility. May perform PT/INR checks in the home as ordered by the Physician and reports results according to policy. What You’ll Do: Reports and documents patient/client’s needs and responses to the appropriate Case Manager and/or Clinical Manager. Reports patient/client’s change in condition or situations immediately. Adheres to patient/client Home Health Aide care plan as set up the primary nurse/therapist while maintaining a healthy, safe environment. Reinforces the teaching of the primary nurse/therapist by reviewing personal care techniques with caregivers and patients. Completes documentation in clinical records, time sheets and quality control logs. Attends a minimum of 12 hours of in-services per year. Provides care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families. Who You Are: Education: High school diploma or equivalent preferred.
Work schedule may include 8 to 12 hours per day with weekend, evening, night, and on-call coverage based on VNA needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the service area of the VNA to meet patient/client needs.
