The provider performs initial comprehensive assessments and develops ongoing patient management care plans for newly enrolled patients, updating them as conditions change or following hospitalization. This role involves prescribing diagnostics, consulting with clinical teams after transfers, and educating patients on disease management and wellness promotion.
Candidates must possess a current state RN/NP license and certification through AANP or ANCC, along with current active DEA licensure or prescriptive authority. Preferred qualifications include 2+ years of clinical NP experience in settings like IM, Geriatrics, Primary Care, or Home Health Care, along with reliable transportation.
