The Nurse Case Manager will utilize Emergency Department nursing experience to enhance patient care, focusing on anticipating discharge needs for post-surgical patients to ensure timely discharge and resource access. This role requires assisting with understanding medical necessity, level of care, and patient status, alongside knowledge of community resources.
Required qualifications include a Bachelor's Degree in Nursing, three years of relevant clinical nursing experience, and being licensed as a Registered Nurse in Wisconsin or a compact state. Preferred qualifications include a Master's degree in Nursing or a healthcare-related field and recent experience as a hospital-based Nurse Case Manager.
