The provider will deliver full-spectrum primary care to patients of all ages, managing chronic conditions, performing preventative care, and addressing acute health concerns. This role requires close collaboration with physicians and clinical staff to ensure coordinated, high-quality care and accurate documentation in the EMR.
Candidates must be a licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Illinois and hold the relevant national certification. Experience in primary care, family medicine, internal medicine, or urgent care is highly desirable, and bilingual skills in Spanish or Polish are a plus.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
