The Case Manager assesses, plans, implements, coordinates, monitors, and evaluates healthcare options and services for individuals. They work collaboratively with patients, families, and healthcare providers to develop and execute effective healthcare plans.
Candidates must have a Master's in Social Work or an R.N. with a Master's in a Health Related Field, along with at least two years of experience in a health-related setting. Excellent communication, planning, and relationship-building skills are essential.
The Case Manager assesses plans, implements, coordinates, monitors and evaluates options and services to meet individual healthcare needs. Promotes quality and cost-effective outcomes.
Bachelor's Degree in Nursing (BSN) is required.
