The Family Nurse Practitioner provides comprehensive primary care services for patients of all ages, including assessment, diagnosis, treatment planning, and education. They also perform minor outpatient procedures and consult with other healthcare professionals as needed.
Candidates must be graduates of an accredited Nurse Practitioner Program and hold current licensure and certification as an Advanced Practice Registered Nurse. Additional certifications in specialty areas and Basic Life Support are also required.
Provision of comprehensive primary care services for all ages in a primary medical care clinic setting. Provide care consistent with evidence-based medicine. Perform minor outpatient procedures in accordance with the scope of practice and privileging policy. Provide consultation to other clinic providers either in person or by telephone. Consult with clinic physicians, specialists, or other health care professionals as appropriate. Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals. Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow-up recommendations as required by uniform data system, meaningful use and patient centered medical home standards. Dispense medications in accordance with State of Mississippi Pharmacy Board policies. Participate in a shared call system with other providers. Assist with meeting clinic patient care team productivity and quality care standards. Participate in development and utilization of practice guidelines and procedures. Participate in audits, and meet standards of Quality Improvement, Risk Management and Utilization Management programs. Complete accurate and timely documentation in patient records and encounters within 24-48 hours after the patient visit. Maintenance of credentials essential for practice, including licensure, certification and CME. Demonstrate respectful, professional and appropriate behavior that supports a team-oriented work environment. Demonstrate a commitment to the mission, core values and goals of Mallory CHC and its healthcare delivery within our programs and services. Participate in the health promotion/disease prevention activities required by the U.S. Public Health Services. Attend all medical staff meetings and participate in the Community Health Center’s Quality Assurance Program. Perform other necessary duties as required by the Community Health Center to meet the goals of providing primary health care. Qualifications Education and/or Work Experience Requirements: Graduate of an accredited Nurse Practitioner Program (i.e., Master’s or Doctorate) Certifications/Licenses: Current Advanced Practice Registered Nurse (APRN) Licensure Current Nurse Practitioner certification in specialty area Current AANP or AANC certification Current provider card in Basic Life Support (BLS) Physical Requirements: Activity Occasionally (1-33%) Frequently (34-66%) Continuously (67-100%) Sitting X Standing X Walking X Bending X Squatting X Climbing X Kneeling X Twisting X Lifting (0-50 lbs.) Able to handle emergency or crisis situations. Occasionally subject to irregular hours. May be required to wear protective equipment as necessary. Travel Requirements: Occasional travel to other clinic sites and as necessary for optimum performance of duties and responsibilities.
