The Nurse Practitioner will manage patient care for CKD and ESKD patients, collaborating with an interdisciplinary team to provide exceptional clinical care. They will also serve as an advocate and navigator for patients, ensuring alignment of treatment and care plans.
Candidates must have a Master's degree in Nursing and at least 2 years of experience as a Nurse Practitioner in relevant fields. Current state licensure, national board certification, and a DEA license are also required.

$120,000.00 - $151,000.00. **This range is inclusive of a geographic differential if located within 50 miles of the Chicago Metro area. This position is also eligible for a target annual bonus of 10%
