The Social Worker will work directly with discharge planners, family, or responsible parties to initiate facility tours and manage follow-up procedures, particularly within the Memory Care Unit. Responsibilities also include developing and maintaining rapport with internal staff, other departments, and external health and social agencies to ensure social service programs meet patient/resident needs.
Candidates must possess a minimum of three years of experience working in a long-term care facility and must hold a Dementia Certification. The ideal applicant needs strong organizational and prioritization abilities, excellent communication skills, and reliable personal transportation.

USD $30.00 - USD $33.00 /Hr. Wage Commensuration The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
Assist in the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents Keep abreast of current federal and state regulations as well as professional standards Qualifications & Requirements Must have at least 3+ years of experience in a long term care facility Must be able to organize and prioritize many tasks effectively Ideal candidate must be a skilled communicator Reliable transportation required
