The Medical Case Manager II coordinates and delivers comprehensive case management services, including eligibility screening, care planning, and resource linkage, to clients impacted by infectious diseases. This role involves collaborating with healthcare teams, performing comprehensive needs assessments, and ensuring compliance with all relevant local, state, and federal guidelines.
Candidates must possess a Bachelor's degree in social work, Nursing, Public Health, or a related field, or have 4-7 years of equivalent experience, along with three years of experience in HIV prevention or intervention. Applicants must also be able to obtain MCM Certification within 90 days, possess a valid driver's license, and pass required background checks.
Description CAN Community Health is now hiring a Medical Case Manager II (Mohave County/Las Vegas) Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm Travel Requirement: Must be willing to report to Mohave County satellite location at least 3 days per week and travel to the Las Vegas clinic for support on flex days.
Salary: $23-$26 per hour based on experience

Education/Professional: Bachelor’s degree in social work, Nursing, Public Health, or related field or 4/7 years’ experience in same fields.
