This role supports inpatient care coordination and discharge planning for medically complex patients in a fast-paced hospital environment.
POSITION OVERVIEW The RN Case Manager is responsible for coordinating patient care across the continuum, ensuring safe and timely discharges, and collaborating with interdisciplinary teams including physicians, social services, and external providers.
