The provider will perform comprehensive patient evaluations, diagnose and treat primary care conditions, manage chronic diseases, and provide urgent care services. Key duties also involve coordinating care transitions, educating patients, and monitoring value-based patients using HEDIS measures.
Candidates must possess a Medical degree (MD or DO) and hold a valid Florida medical license, along with Board certification in Family Medicine or Internal Medicine, and current CPR certification. A minimum of 2 years of primary care experience, especially with value-based care and HEDIS, is preferred, and bilingual proficiency in English/Spanish is highly favorable.

We are looking for a full-time provider who is passionate about primary care and committed to professional growth within our expanding practice.
