The Hospice RN Case Manager will provide exceptional support to patients and their families during end-of-life care, working primarily within senior living communities and patients' homes. This role involves traveling to various locations in surrounding counties to deliver patient-centered care with dignity and comfort.
Candidates must hold a valid nursing license and possess a passion for compassionate end-of-life care, with experience in hospice or home health being preferred. The ideal applicant should be skilled in building relationships with patients and families while maintaining a positive and compassionate attitude.
