The Registered Nurse will perform comprehensive patient assessments in the home setting, establish care plans, and serve as the first point of connection for patients entering Home Health services. Essential functions include delivering coordinated, safe, and evidence-based quality care based on patient needs and completing comprehensive OASIS assessments.
Candidates must be a Registered Nurse licensed in the state of employment, preferably with one to two years of experience, though less experience is acceptable for NLRN Program graduates. A BSN is preferred, and BLS certification is required, along with reliable transportation for daily travel within the service area.
