$65 - $110 / HOUR
The Registered Nurse Case Manager will provide skilled nursing care to home health patients, which includes conducting admission and OASIS assessments and performing head-to-toe and wound evaluations. Responsibilities also involve creating individualized plans of care, providing follow-up visits, educating patients and caregivers, and coordinating care with the interdisciplinary team.
Candidates must possess a current Arizona RN license and reliable transportation, along with excellent communication and interpersonal skills. Prior home health experience and residency in Phoenix or the East Valley are preferred qualifications.
