The Nurse is responsible for the care and treatment of individuals with mental health or intellectual/developmental disabilities, including completing health appraisals, assisting physicians, and monitoring physical health conditions. Key duties involve administering and monitoring medications accurately, training staff on physician orders, and educating individuals and coworkers on various health matters.
Candidates must possess a valid Licensed Practical Nurse (LPN) license or a valid Registered Nurse (RN) license in the state of Pennsylvania. Additional requirements include having a valid driver's license, a good driving record, and access to a reliable vehicle for necessary travel.

The Nurse works in the care and treatment of individuals with a mental health (MH) diagnosis or an intellectual and developmental disability (IDD), which may include physical health issues/injuries and difficulty meeting their daily living activities while participating in services. The Nurse reports directly to the Program Director or designee.
Completes health appraisal on individuals when admitted to program. Provides assistance to physicians as needed. Evaluates, observes, and assesses physical health conditions. Assists individual with any treatments prescribed by a physician. Assists in administering/monitoring medications, while ensuring individuals receive error free medication at prescribed times. Monitors Vitals and intakes and outputs. Administers injections per physician orders. Trains employees to correctly carry out physicians orders. Educates individuals on medication, physical health, psychiatric health, nutrition, and selfcare. Educates coworker’s on any needed physical health matter for consistency in care. Works with and coordinates care with pharmacy, physicians, and other health care providers. Assist/manages medication refills, as needed. Obtains medication and treatment needed from pharmacy or medication samples if available. Completes medication progress sheets daily if regulated by program. Completes medication release forms for discharge, as needed if regulated by program. Completes pill and/or narcotic counts on established schedule if required by program. Completes and facilitates medication disposal, as needed. Completes medication records i.e., Medication Account Records, Physician Order Sheets, Nursing Tracking Forms etc. as per established schedule. Completes medication record audits as per established schedule. Schedules and attends individuals’ medical appointments. Drives van to appointments and outings and assists individuals in and out of the van. Assists individuals in community settings. Provides or directs the provision of nursing care in accordance with the nurse practice act and legal responsibilities as defined by the State Board of nursing. Facilitates weekly Wellness Groups for residents. Conducts pre-evaluation for psychiatric evaluation as needed. Willing to learn and model the five principles of trauma informed care: safety, choice, collaboration, trustworthiness, and empowerment. Commitment to promote cultural, racial, language and gender access, diversity, equity and inclusion. Provides direct care as needed. Knowledge of the uses and effects of psychotropic drugs and narcotics, safety practices and location security. Knowledge of common illnesses, diseases, disabilities, injuries, and nutrition. Ability to collect data and document in the individual’s record according to policy and procedures, from individual’s admission and throughout the stay and communicate relevant information to the program director. Ability to utilize universal standards when providing individual care. Specific to the Adolescent Residential Treatment Facility (RTF): Ability to learn the theories, principles, and techniques of group and individual psychotherapy.
This position requires one of the following combinations of education and experience: A valid Licensed Practical Nurse (LPN) license or a valid Registered Nurse (RN) license in the state of Pennsylvania. Additional requirements include: Must have a valid driver’s license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.
