The Case Manager provides essential case management and housing support to assigned families, conducting needs assessments, home visits, and arranging necessary services and referrals. Key duties also involve completing required documentation, coordinating workshops, and assisting clients with housing applications like Section 8 and NYCHA.
Candidates must possess a Bachelor's degree in a relevant field, with a Master's degree preferred, alongside a minimum of two years of Case Management experience. Essential qualifications include experience working with homeless populations, knowledge of housing procedures, and familiarity with community resources.

Under supervision of the Director of Social Service, the Case Manager is responsible for providing case management, housing support to clients and monthly reporting requirements that will ensure compliance with the parameters of the program. The requirements listed below represent the knowledge, skills, and/or ability required.
The salary for the Case Manager is $40,000 - $45,000/annually based on relevant experience.
