The role involves providing neurobehavioral health services to elderly residents in long-term care facilities, utilizing a multi-disciplinary approach to offer holistic care for those with neurocognitive and psychiatric conditions. Responsibilities include conducting evaluations, providing condition-specific or insight-based therapy, and coordinating care plans with facilities, PCPs, and families.
Candidates must possess a Master's degree in Mental Health Counseling or Social Work and hold current LCSW or LMHC licensure in the state of application. Required attributes include the ability to function autonomously within a team structure and strong time management skills.
As a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.
