Candidates should possess at least two years of professional experience in Senior Housing, Multi-Family housing, or a non-profit social service organization, with a preference for a bachelor's degree in a related human services field. The RSC will establish and sustain partnerships with community-based social service providers and other stakeholders to assist and empower residents to meet their needs through education, training, and accessing services. The RSC will assess residents’ individual needs for social service intervention, either on referral by property management or by residents’ self-referral. The RSC will determine and coordinate services with third-party (non-profit) social service providers. Where possible, monitor resident outcomes (change in circumstances), documenting all contact with residents and referrals in case files/case notes. The RSC will seek to establish strong community relationships to help residents to identify and effectively manage their social service needs. The RSC will create and sustain partnerships with community-based social service providers and other community stakeholders. Essential Functions Develop strategic partnerships with local service providers to bring on-site programs or provide referrals to off-site programs and services in the following outcome areas: education, health and wellness, workforce development, financial literacy and stability, transportation, food and nutrition, personal care services, and asset building. Provide non-clinical case management and referral services to all residents, including securing the full range of social services that individual residents need. Initiate strategic relationships with other local service providers and stakeholders. Build relationships with residents and their families to understand their needs and aspirations better and connect them with local resources and opportunities. Acts as a liaison between residents and Property Management, community-based agencies, and/or regional government entities (Social Security Administration, Dept. of Veteran’s Affairs, Adult Protective Services, etc.). Works with Property Management staff, eldercare service providers, behavioral/mental health care, and medical care professionals to resolve issues while keeping information confidential. Monitors, documents, and helps residents prevent and manage cognitive, emotional, and physical health issues, including ADL limitations, or, in rare cases, assists residents with their move to a care facility and coordinates transportation services for the residents. Create consistent outreach and communication with residents, including newsletters, flyers, telephone calls, and home visits. Maintains accurate and timely documentation on all resident demographics, issues, requests, statistics, incidents, interactions, and outcomes through services software. Align initiatives with Vesta's resident services program's outcomes, mission, and goals. Participation and coordination of resources around critical issues such as housekeeping and money management for late rent payers. Manage administrative tasks required for securing and executing contracts with providers and partners, such as Memorandum of Understanding. Develops and maintains linkages with area resources, including 3rd party providers, regarding information and services available to address the social and recreational needs of the community and individual resident needs. Other duties and responsibilities as requested. Knowledge, Skills, and Abilities: Ability to understand and respect values, attitudes, and beliefs that differ across cultures and to respond appropriately to these differences with residents in planning, implementing, and evaluating programs and services. Capable of building and maintaining positive relationships with various stakeholders, including but not limited to residents, staff, volunteers, interns, and community partners. Experience working with seniors and disabled individuals to support their aspirations. Related human services/social services experience, possessing knowledge of the aging services network, aging processes, and eldercare service delivery system. Ability to collect, track, and understand data to assess programs and partnerships and inform strategies. Operate in a timely manner, with consistency and a high level of integrity and professionalism; provide excellent customer service to residents and partners. Excellent communication, organization, and writing skills. Knowledge of federal, state, and local housing policies is a plus. Ability to work some weekends and evenings is required Interested? Required Education & Experience: Bilingual English and Spanish verbal and written communication skills preferred. Preference for a bachelor’s degree focusing on social work, human services, public health, psychology, Gerontology, Developmental Disabilities, community development, or a related field. 2+ years of overall professional experience working in a Senior Housing/Multi-Family housing environment or a private non-profit social service organization. Experience overseeing and managing grants a plus. Proficiency with MS Excel, PowerPoint, Word, Outlook, and other computer programs. Previous experience in property management, and knowledge of HUD regulation a plus. Previous engagement, communication, and collaboration with residents of diverse socioeconomic and cultural backgrounds are preferred. Experience working with partners and site team members to implement programs and initiatives. Successful completion of a background check and drug screening required.
