The Palliative Social Worker is responsible for providing psychosocial care to clients and families, assessing their emotional, social, and economic needs. They also participate in interdisciplinary team meetings and assist in developing comprehensive care plans.
Candidates must have a Master's Degree in social work and a current social worker license in good standing. A minimum of one year of social work experience in a healthcare setting is required, with a preference for two years in varied multi-disciplinary settings.
Palliative Medical Social Worker Full Time Monday-Friday 8-5 NO WEEKENDS and NO ON-CALLS!! Mileage Reimbursement PURPOSE According to prescribed policies and procedures of the organization including all applicable federal, state and accreditation regulations and under the general supervision of the Director of Clinical Management is responsible to provide clients and families psychosocial care regarding emotional, social and economic needs. ESSENTIAL FUNCTIONS Assesses the psychosocial status of clients related to the patient’s illness and environment and communicates findings to the registered nurse and/or physician(s). Provides services that are ordered by the physician as indicated in the plan of care and carries out social evaluations and plans intervention based on evaluation findings. Maintains clinical records on all clients referred to social work. Provides information and referral services of organization clients and families/caregivers regarding practical and environmental needs. Provides education to and serves as a liaison between clients or families/caregivers and community agencies. Participates in the development of the total plan of care including case conferences and discharge planning, as required. Assists physician and other team members in understanding significant social and emotional factors related to health problems. Participates in QAPI program and HHA sponsored in –service training, as required. Participates in interdisciplinary team meetings to coordinate the care of the client and family; exchange information and problem solve; and receive staff support and education. NON-ESSENTIAL FUNCTIONS Maintains collaborative relationships with organization personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel. EXPECTATIONS Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem solving methods. Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook, including but not limited to, corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws. With respect to Resident Rights, ensures all care is provided with respect and dignity for the residents/clients, their families and the staff. Reports all complaints made by residents/clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state and local regulations and laws. Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties. Performs other duties as assigned. JOB SPECIFICATION SHEET –Medical Social Worker Education: Master’s Degree in social work from a school of social work accredited by the Council on Social Work Education.
Preferred minimum of two (2) years in varied multi-disciplinary settings in the health care field with experience working with clients and family counseling; however, requires one year of social work experience in a health care setting.
Must have a current social worker license in good standing.
