The Patient Service Line Navigator acts as a primary point of contact to coordinate care for heart failure patients across hospital and outpatient settings. They provide patient education, manage referrals, and collaborate with the multidisciplinary team to ensure optimal clinical outcomes and safe transitions.
Candidates must hold a Bachelor's in Nursing, a Master's in Social Work, or a Master's in Health Administration, along with at least three years of related work experience. A valid Registered Nurse license in Florida and BLS certification are required, with ACLS certification preferred.
Depending on the needs of the service-line area, must have (1) a Bachelors in Nursing (BSN) from an accredited School of Nursing (or must obtain BSN within 2 years of hire) with licensure to practice as a Registered Nurse by the State of Florida, or (2) a MSW degree from a CSWE accredited college or Social Work program or (3) a Master’s degree in Health Administration or related field. Three years related work experience required. BLS required.
