Weekend LPN (Licensed Practical Nurse, Visting Nurse) - HomeCare

• As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care.
• This role will collaborate with all team members to achieve exceptional outcomes.
• Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
• Records daily progress in EMR.
• Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences with primary nurse and other disciplines.
• Consults with physician regarding client needs and plan of care.

Minimum Education Requirement

High School diploma or GED - Successful completion of a CT state approved LPN certificate program Minimum Licensure, Certification, Registration - Current and valid LPN license - Valid CT driver’s license & a good driving history
