The Intensive Case Manager provides support to the re-entry population in Los Angeles County to prevent recidivism and secure permanent housing. Responsibilities include conducting assessments, developing treatment plans, and coordinating with mental health and legal partners.
Candidates need a Bachelor's degree in behavioral health/social science with one year of experience, or a high school diploma with three years of relevant experience. A valid driver's license, vehicle, and insurance are required for client transport and site visits.
The intention of our services is to help people in Los Angeles County who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism. The role of the Intensive Case Manager is to help create a supportive network of services and resources for the client to prevent reincarceration and support long term permanent housing goals. \n
Interview and assess participant needs through interviews with participants referred by the Office of Diversion and Reentry. Conduct wellness checks at the client’s homes and provide crisis management for any issues. Develop a treatment plan with the participant to identify and address service needs and goals. Assist with public benefits advocacy and linkage. Coordinate and link the client to needed mental health, substance abuse, and housing resources. Work with other staff to assist with placement and case management upon release. Collaborate with housing staff, probation officers, psychiatrist, public defenders, primary care providers, and other partners with the client to ensure a coordinated effort to best serve the participant. Attend inter-agency case coordinated meetings, coalitions, regional advocacy groups, and boards in benefiting the participants served and support community cooperation in addressing areas of need. Maintained concise and comprehensive case records on all participants seeking services. Provide accurate daily and monthly documentation and case notes as per funding source guidelines. Required to use company vehicle as well as personal vehicle to visit county jails and transport clients to and from appointments and other locations as needed.
Bachelor’s degree in behavioral health or social science and one year of experience providing outreach, substance abuse counseling, OR The equivalent of a high school diploma and three cumulative years of experience of administrative responsibility in providing outreach, substance abuse counseling, reentry services, and case management within a culturally diverse population. Knowledge on how to administer assessments, substance abuse counseling, reentry services, and case management within a culturally diverse population, preferably with both men and women. Excellent organizational skills and attention to detail. Excellent written and oral communication skills. Extensive knowledge of non-profit organizations. Proficient in Microsoft Office Suite or similar software. Required to have car insurance, vehicle, and valid driver’s license.
