The Case Manager provides supportive case management services to individuals with mental illness in the apartment treatment program, including monitoring site maintenance and planning/evaluating service plans weekly. Responsibilities also involve overseeing medication maintenance, assisting residents with daily living skills, providing crisis management, and coordinating with families and providers.
Candidates need either a High School Diploma with two years of qualifying experience or a Bachelor's degree with one year of qualifying experience, where qualifying experience involves case management with populations dealing with mental illness, homelessness, or chemical dependence. A valid driver's license, safe driving record, valid auto insurance, and access to a vehicle are mandatory.

Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.
$40,040/year ($22.00/hour) - $50,960/year ($28.00/hour)
