Provides case management services to
ACT program clients evidenced by: Assisting in the development of and functioning as the coordinator of the comprehensive treatment/service plan in a manner that is person-centered and meets both agency and regulatory expectations. Providing ongoing interventions, education, resource linkage and skills coaching in individual or group format related to symptom management, independent living, community activity, interpersonal interaction and socialization, and daily living skills. Assisting individuals in accessing resources and education related to basic needs such as benefits, food/nutritional support, financial support/education, transportation, and stable housing. Assists and supports Participants to have and effectively use a primary care physician, dentist, and other specialists as required. Organizes and leads individual and group social and recreational activities to structure enrolled participants’ time, increase social experiences, and provide opportunities to practice social skills Demonstrating the ability to recognize and assess suicide potential and risk, and provide appropriate responses to the situation. Supporting individuals served and their families in crisis situations. Provides on-call crisis intervention as scheduled. Plans, structures, and prompts social and leisure-time activities on evenings, weekends, and holidays as assigned.
