Facilitates the patient's hospitalization process from admission to discharge by coordinating with a multidisciplinary care team. Manages resource utilization and provides health education to ensure cost-effective, quality patient care.
Requires a nursing degree from an accredited school and a current Registered Professional Nurse license in New York State. A Bachelor's degree in Nursing is preferred, with a requirement to obtain one within five years of hire.
Facilitates the patient’s hospitalization from admission through discharge. Coordinates with physicians, other nurses and social workers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of care.
