Provide specialized assistance to patients in gathering documentation and completing applications for public benefits and Social Security. Coordinate with healthcare providers and public entities to secure medical evidence and support the claims process.
Requires 2 years of experience in health education or social services and a valid California driver's license with a reliable vehicle. A bachelor's degree in a related field is preferred along with familiarity with Social Security Administration impairment listings.
Provide dedicated and specialized assistance for patients and clients. Client base consists of referrals from FHCSD’s Enhanced Care Management, Community Support Team Members, and PACE. Provide assistance with client base in gathering information to complete applications and filing claims/appeals. Gather medical records and other needed information and documentation in completing applications. Complete and write SOAR Medical Summary Reports for each individual’s application. Accompany individuals to set appointments at the Social Security Administration as appropriate through each element of the required process. Provide coordination related to visits to medical doctors, psychiatrists, and other healthcare specialists in obtaining evidence in support of application process. Serves as a point of contact with public benefits entities on behalf of FHCSD patients and clients. Performs other duties as assigned Requirements: A Bachelor’s degree in a related field preferred. Evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness. Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency. Mileage and other reimbursement governed by policy. 2 years of experience with health education or working in the medical/health or other social services related field. Familiarity with Social Security Administration’s Listings of Impairments. Ability to prioritize multiple tasks and meet frequent deadlines. Superior organizational skills and attention to detail. Good writing skills and the ability to analyze extensive data and create written reports with accuracy and brevity. Ability to maintain professional boundaries and engagement skills with a challenging population and in non-traditional work conditions. Strong verbal and written communication skills, including the ability to communicate effectively with those who speak English as a second language. Computer experience, including troubleshooting and problem solving and proficiency in MS Office applications (Word, Excel) and database management.
