The Social Worker 2 collaborates with the healthcare team, patient, and family to coordinate timely psychosocial assessments and formulate patient-centered discharge plans. This role involves providing psychosocial assessment and intervention, developing comprehensive plans based on identified needs, and linking patients to community resources.
A Master's Degree from an accredited school of Social Work is required, along with proficiency in computer skills, theoretical/clinical healthcare delivery knowledge, multitasking, autonomy, and effective communication. Preferred experience includes work in a hospital or health agency, and a Licensed Independent Social Worker (LISW) certification is preferred within one year of eligibility.

This position will attend to activations, addresses discharges and assessment needs in the TSICU, offers psychosocial support, and also supports discharge of floor patients as needed. The schedule is Mon-Fri, 3P-11P shift, but occasional weekends and holiday coverage is required.
