The physician will perform comprehensive in-person assessments and lead/partner with the interdisciplinary team to create personalized care plans for each participant. Essential duties also include coordinating discharge for acute, short-term, and long-term placements, focusing on comprehensive care with an average of 8 patients daily.
Candidates must successfully complete an accredited residency program and hold a Master's or doctorate degree, with Board Certification being preferred. Active state physician license without restrictions, State CDS certification, and DEA registration are mandatory, along with obtaining CPR and First Aid certification shortly after hire.

Our value-based PACE program allows our Primary Care Physicians to provide truly comprehensive, holistic care, so you can focus on what really matters: your patients.
We are seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics. Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants. Board certification preferred.
