The Home Health Aide will provide personal care and related services, focusing on activities of daily living, in the home for sick or disabled patients. Responsibilities include assisting with personal hygiene as per the care plan, maintaining infection control, and documenting patient records.
Candidates must possess a Home Care Aide certificate and maintain an automobile with a valid driver's license and proof of insurance. A minimum of one year of full-time direct client care experience in an institutional setting and one year in an agency setting within the last five years is required.
The Home Health Aide provides personal care and related services, focusing on activities of daily living, in the home for the sick or disabled. As a Home Health Aide you will: Provide assistance with activities of daily living (i.e., personal hygiene) as part of the agreed-upon plan of care Understand and apply basic infection control principles and procedures Keep a record of HHA Plan of Care for each patient assigned and provides updates to assigned case manager and patient’s family Attend required in-services and meetings Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
