This role involves providing direct non-professional patient or resident care under the supervision of licensed nursing personnel, including assisting with activities of daily living and maintaining a positive environment. Responsibilities also include coordinating unit activities like admissions, transfers, and discharges in the Transitional Care Unit.
Candidates must conduct regular care rounds to monitor residents, assist with daily needs, and promptly notify the Charge Nurse or RN supervisor of any condition changes. Essential duties include obtaining vital signs, completing all assigned documentation, and ensuring compliance with resident rights and safety protocols.
