The Physical Therapist will conduct patient evaluations and develop individualized care plans to restore mobility and independence. They will implement treatment plans and collaborate with an interdisciplinary team to ensure comprehensive patient care.
Candidates must be graduates of an accredited Physical Therapy program with an active California license and current CPR certification. A minimum of one year of experience in a healthcare setting is preferred, particularly in home health.

In this role, you’ll help patients regain mobility and independence in the comfort of their homes — working with a supportive interdisciplinary team that values collaboration, flexibility, and clinical excellence. The Physical Therapist provides evaluation and therapeutic services in the home under a plan of treatment. Care is directed at rehabilitation to maximum potential, as well as promotion of independence. Teaching patients rehabilitative and therapeutic procedures while in their home setting is a powerful addition to successfully meeting recovery plans.
