The Social Worker will be a vital member of the patient care team, supporting and advocating for patients undergoing the difficult transition of life on dialysis. This role involves guiding patients in managing the physical, mental, emotional, and financial demands associated with End Stage Renal Disease.
Candidates must possess a Master's Degree in Social Work (MSW) with a clinical specialization and hold the required Clinical License (e.g., LCSW, LICSW) for the state. Proficiency in experienced level Social Worker duties, intermediate computer skills, and demonstrated knowledge of government and private insurance programs are also mandatory.
