The Care Manager acts as the primary contact for program participants, shaping their view of services by providing care management, outreach, engagement, and support to youth in supportive housing. Key duties involve ensuring participant safety and well-being while addressing co-occurring challenges like mental health, domestic violence, and substance use.
Candidates must possess a Bachelor's degree in a social service-related field along with a minimum of three years of experience working with children, youth, or families. A valid driver's license with an acceptable record is mandatory, alongside the ability to exercise independent judgment and manage time effectively.

Base rate: $20.67/ hourly Bilingual Rate: An additional $1.50 added to the base rate if Spanish proficient. Sign-on Bonus: $500 to welcome you to our team!
