The Therapist provides integrated clinical services focused on adults with schizophrenia-spectrum disorders through direct therapy, case coordination, and recovery support to enhance stability and community integration. This role involves delivering person-centered treatment, coordinating care across various systems, and actively supporting clients in maintaining housing stability.
A Master's Degree in Social Work, Counseling, or Marriage and Family Therapy is essential, along with an active LSW or LPC license in Ohio; an LISW or LPCC license is preferred. Candidates must be proficient in providing evidence-based therapeutic care tailored for serious mental illnesses.
Provide individual and group therapy using evidence-based practices tailored to individuals with schizophrenia and other serious mental illnesses. Conduct comprehensive mental health assessments, develop individualized treatment plans in collaboration with the client, and care team. Deliver services with a recovery-oriented, trauma-informed, and person-centered framework that emphasizes client autonomy and community integration. Coordinate care across behavioral health, medical, and housing systems, acting as a liaison among providers, case managers, housing staff, and families when appropriate. Support housing stability to addressing behavioral health needs to impact tenancy including crisis intervention, symptom management, and medication adherence. Collaborate with housing staff and property management to ensure clients maintain housing and receive appropriate wraparound service. Maintain accurate, timely documentation in compliance with agency, funding, and licensing standards. Engage in regular interdisciplinary team meetings to ensure continuity of care and shared decision-making. Provide crisis intervention and risk assessment, including suicide and violence risk, and coordinate higher levels of care as needed.
