The Case Manager supports clinical and non-clinical staff by providing auxiliary services, conducting initial assessments, leading group counseling sessions, and developing comprehensive aftercare plans for clients in recovery. Key duties involve weekly patient meetings, meticulous documentation, facilitating referrals, and coordinating discharge surveys for data aggregation.
Candidates must possess a High School Diploma, a valid driver's license with a clean record, and a Fingerprint Clearance Card, alongside strong interpersonal skills and experience in case management. Proficiency in Microsoft Office and familiarity with Electronic Medical Records systems are also required.
Experience in detox, residential, or inpatient medical/behavioral health preferred. Experience performing intake, assessment, treatment planning, and coordination of multi‑disciplinary services. Familiarity with care coordination involving medical providers, therapists, MAT programs, probation/parole, and community resources. Knowledge of withdrawal management, substance use disorders, co‑occurring disorders, and relapse‑prevention support.
