The Clinical Rehab Navigator will establish relationships with healthcare providers and payors to drive patient admission volume for the inpatient rehabilitation unit. They will also perform clinical chart assessments to ensure appropriate patient selection and provide educational resources to patients and their families.
Candidates must hold a valid license as a Registered Nurse, Physical Therapist, or Occupational Therapist in Florida. The role requires a minimum of a high school diploma with two years of experience, or an associate/technical degree.

One or more of the following required: Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date. Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Florida Board of Physical Therapy Practice obtained prior to hire date or job transfer date. Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Florida Board of Occupational Therapy obtained prior to hire date or job transfer date. BLS Provider preferred. American Heart Association or American Red Cross accepted. Drivers License preferred.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
