The SUD Case Manager will deliver intensive case management services to incarcerated adults in multiple jails, focusing on successful community re-entry and reducing recidivism for at least 30 days post-release. This role requires providing holistic care supports including housing, employment, healthcare, and relapse prevention to ensure individualized success in the community.

Candidates should possess at least one year of experience in a behavioral health field and must meet all Ohio Chemical Dependency Professionals Board standards, including competency in working with the served populations.
The mission of this position and grant program is to improve the probability of successful re-entry from jail into the community for incarcerated adults in the Logan County Jail, Tri-County Regional Jail, and West Central Community Correctional Facility and to reduce the likelihood of recidivism. Integrating the person successfully into the community will require a holistic care approach that includes various recovery supports such as housing, peer supports, supported employment services, healthcare and nutrition, transportation, family reunification, childcare, relapse prevention, and spiritual support. The SUD case manager for this program will provide services in the jail while the individual is incarcerated and follow them through re-entry into the community. The SUD Case Manager will work intensely with the individuals and remain involved with them for at least 30 days post jail release to allow sufficient time to connect with appropriate community services. The intent of this position is to provide specific, measurable, and individualized services to each person served. Case management services should be focused on the individual’s ability to succeed in the community including reduction of recidivism, improvement in work and family integration and contributions within the community.
Education: Minimum of High School Diploma- associate degree preferred Certification: CDCA; higher licensure recommended Experience: At least one year in a behavioral health field is preferred Other: CPR; First Aid; De-escalation which will be provided if necessary Meets all standards and regulations of the Ohio Chemical Dependency Professionals Board Must meet all requirements associated with the completion of a Medicaid application and receiving approval to bill for services rendered. Demonstrated competencies for working with the populations served. Valid Ohio Driver’s License, reliable transportation, and appropriate automobile liability insurance.
