Community Health Workers serve as a bridge between members and the healthcare system through outreach and education to help Aetna members achieve better health outcomes and improve provider relationships. This role requires flexibility to work in unknown conditions, communicate medical information, and influence providers regarding member preferences.
Candidates must reside in the specified counties and possess reliable transportation, willing to travel up to 75% of the time, requiring 1-3 years of relevant experience, preferably serving Medicaid populations. A High School Diploma or GED is the minimum education requirement for this full-time field position.

Experience working in community/outpatient setting. Community Health Worker Certification or equivalent training and experience. Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions. Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc. Experience using a computer including the use of Excel and Electronic Health Records Education High School Diploma or G.E.D Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $18.50 - $38.82 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
