The role involves providing personal and homemaking services to ill, elderly, or disabled individuals in their homes, including direct patient care like bathing and hygiene assistance. Responsibilities also include monitoring and recording vital signs, assisting with transfers, and supporting telehealth program functions as directed.
Candidates must possess a high school diploma or equivalent and successfully complete required written competency and clinical skills testing for home health/hospice. Certification as a Home Health Aide (CHHA) is mandatory, and a valid driver’s license with insurance is required for position-specific duties.
Under direct supervision, provides personal and homemaking services to ill, convalescing, elderly, and disabled persons in the patients' home. Essential Functions and Responsibilities: 1. Provides personal care of a patient, including bathing, grooming/hygiene, and toileting. 2. Takes and records daily vital signs. Reports all vital signs that are outside the normal range. 3. Assists patients with positioning/turning, transfers, catheter care, and range of motion exercises. 4. Light housekeeping and food preparation. 5. Recognizes and reports any unusual, untoward, or new symptoms/changes to the RN. 6. May perform functions to support the telehealth program (uninstalls, equipment cleaning and maintenance, installations/setups). 7. Performs other related duties as required and directed.
